Foundation for Community Health Prescription Assistance Program
The Healthcare Consortium is pleased to remind residents of the town of Gallatin that the following program is available to them.
Foundation for Community Health Prescription Assistance Program
The FCH Prescription Assistance Program can help cover costs associated with prescriptions, prescription premiums and co-pays, and the cost of equipment needed to administer medication, such as syringes and nebulizers.
Glucose test strips and glucometers are also eligible for financial assistance.
This program is available to all residents of Gallatin, Ancram, Pine Plains and Copake who qualify. Income must be at or below 400% of the Federal Poverty level.
That means an individual making up to up $54,360 or a couple making up to $73,240 could qualify in 2022. The Federal Poverty guidelines change each year.
